DRV3901-Q1EVM: DRV3901-Q1EVM flashing red fault LEDS

I received the eval board and have had an issue trying to bring it up.  The packaging was not original as it was clear the static bag had been opened and used.  When connecting the USB to power the board I get 2 flashing red “fault” LEDs on the Status LED bank.  I have tried using the downloaded GUI software as well as the online launch-able GUI on two different computers and get the “failed to connect” on all attempts.  Can I get a replacement board or additional support please.
